Title: Initial IV Fluid Bolus in Septic Shock<br/>Author: Kami Windsor<br/><a href='http://umem.org/profiles/faculty/742/'>[Click to email author]</a><hr/><p>
        The Surviving Sepsis Campaign guidelines recommend a 30ml/kg IV fluid bolus based on limited retrospective data.^1,2</p>
<p>
        A recent Chinese multicenter prospective observational study across 4 ICUs stratified patients with septic shock into low (<20ml/kg), medium (20-30mL/kg), and high (>30ml/kg) fluid groups based on the initial fluid bolus given.³</p>
<p>
        Out of 300 patients, those in the high-volume group had the highest 28-day mortality (48.3%), while those in the medium group had the lowest (26.3%, p<0.05). In the medium group, patients completing their bolus within the first hour had the lowest mortality rate of all (22.8, p<0.05).</p>
<p>
        Notably, it was patients in the medium group who had the lowest rate of invasive mechanical ventilation (35.3%, p = 0.01), with the greatest change in SOFA score by day 3.</p>
<p>
        <strong>Bottom Line:</strong> For general patients presenting to the ED with septic shock, an initial bolus of 30ml/kg given within the first 1-2 hours seems to improve outcomes. Avoid iatrogenic fluid overload, as higher volumes are associated with worse survival.</p>
