"Geriatric Trauma: Growing Old is Hard"

• Injury is the 7th cause of death in those > 65. Most frequent mechanism is falls, followed by MVCs, pedestrian struck and violence. The mortality of ground level falls in the geriatric population? 7%. • Ages 70-74 is when mortality sharply increases from trauma. Consider transfer to a trauma center. • Osteoporosis and decreased muscle mass leads to increased risk for fracture and after the injury it become harder to regain function. • The elderly often have decreased cardiac output, decreased sensitivity to catecholamines, and are on medications that blunt the shock response. Vital signs are not reliable. Hypotension in the older patient is considered SBP < 110. HR > 90 is considered tachycardic. • Geriatric patients with head trauma should be imaged. Do not try to apply clinical decision rules as they are not reliable in the population.
