• HR > 140 are likely due to an organic medical etiology and should not be chalked up to anxiety. • Consider urologic consultation for ureterolithiasis cases in which the stone is proximal and greater than 5mm, bounceback presentation, intractable pain/vomiting, concurrent urinary infection. Stone + infection = emergent decompression. • For diagnoses with associated high mortality, such as ruptured AAA, have the conversation regarding goals of care early (as in, in the ED).
