• Document what you do! • The coder reviews your note and assigns current procedural terminology codes (CPTs) based on documentation and ICD 10 diagnoses to assign RVUs. If you don’t document, the coder won’t see it. This includes social, family, and past medical history. • Your final diagnoses can include relevant history, abnormal vital signs, and relevant symptoms that show how complex a case was. • Interpretations of ECGs, x-ray and, cardiac monitoring are separately billable procedures. • Critical care documentation: A patient does not need to be admitted to document critical care time. You must spend at least 30 minutes with patient, on their documentation, talking to family, calling consultants, etc.
