• Multifocal atrial tachycardia (HR > 100) or wandering atrial pacemaker (HR <100) is defined by irregular tachycardia with at least 3 distinct P waves morphologies in the same lead. Treat the underlying cause. • DDx for left-axis deviation: LBBB, LVH, pacemaker, elderly, LAFB, old inferior infarct, WPW • ST elevation in avR with associated depressions is a physiological phenomenon of SVT. If it goes away after they convert to sinus rhythm, there’s nothing to be done! This does not equate to a “failed stress test” • ST elevation in avR differential includes Left Main Coronary Artery Occlusion, Triple Vessel Disease, Proximal LAD. Proximal LAD is suggested if ST elevation in avR + avL. Also suggested if ST elevation in avR > ST elevation in V1.
