Top 10 Things to Know About Ischemic Stroke

• Last known normal is not the same of symptom onset as many will have had symptoms earlier or no one has seen them in a while. • Frank hypodensity on the non-contrast CT should not be given tPA as the risk of hemorrhage is significant. • We are moving toward treating minor strokes or people with improving symptoms as stroke symptoms may be dynamic and even a small deficit may be life-altering. • Consider endovascular therapy in patients with proven large-vessel occlusions and penumbra. • There is little data on whether there is a benefit to suspected CVAs rerouting to a comprehensive stroke center. We are currently studying this in Baltimore City, so be prepared to see more stroke patients in our ED.
