OB/GYN - Cardiovascular Disasters in Pregnancy:

• Disorders of the cardiovascular system is the most common cause of pregnancy-related death (greater than infection/sepsis, cardiomyopathy, venothromboembolism). • Preeclampsia and eclampsia can present up to 6 weeks postpartum. Treat to get pressure < 140/90mmHg using antihypertensives such as hydralazine, labetalol, nifedipine. Seizure control with 4-6 grams IV over 10 minutes. No IV access? Give IM magnesium. • Signs and symptoms of magnesium toxicity include decreased deep tendon reflexes, somnolence, respiratory depression and paralysis. Treatment? Calcium! • DVTs during pregnancy are most likely to occur in the proximal left lower extremity due to uterine compression on the left common iliac. If high clinical suppression despite negative bilateral lower extremity duplex, patient may require MRV to evaluate for pelvic vein thrombosis
