• ECG with STEMI with non-classic history think about dissection dissecting into the coronaries. • Esmolol is good for heart rate control as it has a short life and easily titratable. Can also use labetalol or diltiazem for heart rate control. • Control heart rate first to avoid rebound tachycardia aiming for SBP ~100 mmHg. Use clevidipine, nicardipine, or nitroprusside for blood pressure control. • If you are doing a pericardiocentesis for tamponade take off just enough to improve the blood pressure. Don’t' completely drain as it may pull more blood into the pericardial sac.
