• There is little evidence for giving a single dose of IV antibiotic and then discharging with PO antibiotics. Just give PO. • Download the free UMMC version of the Sanford guide (ask Ashley if you need help) which includes our institution specific antibiograms to help guide your antibiotic choices. • In a sick patient with limited IV access give the piperacillin-tazobactam 1st. It infuses much quicker than vancomycin and offers broader coverage.
