• Secondary syphilis rash most commonly starts on trunk or flexor extremities and spreads to involve palms and soles. • Clinical suspicion and spinal fluid examination (CSF VDRL, CSF WBC, CSF protein) are keys to the diagnosis of neurosyphilis. • Jarisch-Herxheimer reaction is a transient febrile reaction to therapy (penicillin) owing to antigen liberation from spirochetes. Symptoms also include rash, headache, malaise. • Standard treatment for tertiary syphilis (penicillin 2.4 million U IM once weekly for three weeks) is not adequate for treatment neurosyphilis or ocular syphilis.
